Odinma Health collects and maintains health information to provide safe and effective care. HIPAA requires us to protect your information and inform you how it may be used or disclosed.
How Your Information May Be Used or Disclosed
1. For Treatment, Payment, and Healthcare Operations
- Treatment: Coordination and communication with other providers involved in your care.
- Payment: Insurance verification, billing, claims processing, and eligibility checks.
- Operations: Quality improvement, training, administrative tasks, record maintenance, and compliance activities.
2. When Required or Permitted by Law
We may disclose information without your written authorization for:
- Public health reporting (disease control, adverse events, etc.)
- Abuse, neglect, or domestic violence reporting
- Health oversight audits and investigations
- Court orders, subpoenas, or administrative proceedings
- Law enforcement purposes
- Identification of deceased individuals
- Funeral or organ donation processes
- Approved research
- Prevention of a serious and imminent threat to health or safety
- National security or military purposes
- Workers' compensation claims
- Contacting family or caregivers when necessary for your care or in your best interest
Some records, including HIV status, genetic testing, substance use treatment information, and certain mental health records, have additional protections under state and federal law.
3. Uses Requiring Written Authorization
Your written authorization is required for:
- Psychotherapy notes (when applicable)
- Marketing purposes
- Any sale of protected health information
- Any use or disclosure not already described in this Notice
You may revoke authorization at any time in writing.
Your Rights Regarding Your Health Information
You have the right to:
- Access or request copies of your medical records
- Request corrections to inaccurate or incomplete information
- Request restrictions on certain uses or disclosures (some requests may be denied)
- Request confidential communication by alternate means or locations
- Receive notification if a breach of unsecured health information occurs
- Receive a paper copy of this notice at any time
You may file a complaint directly with Odinma Health or with the U.S. Department of Health and Human Services if you believe your privacy rights were violated. You will not face retaliation.
